Please submit all materials through website portal before September 1.
Proposal
- A brief history of the organization, including its mission
- A statement of need, and goals, for the program or project
- A description of “means” to achieve the stated goals
Other Attachments
- Program/project budget, including revenue and expenses. Under review please provide other foundations you have applied to for funding
- Copy of IRS 501(c)(3) letter of determination
- Most recent audit or IRS 990
- Optional – Brochure, newsletter or written material describing program
Upon receipt of the completed proposal, the Foundation may request additional information.
